Long Term and Chronic Conditions

The practice is proud to offer a comprehensive range of primary care treatments and services, including annual comprehensive reviews for patients with chronic diseases, such as:

  • Heart disease

  • Asthma

  • Hypertension

  • Diabetes (including appointments with dietician and podiatrist where needed)

We also regularly review our patients with mental health diagnoses, those with learning difficulties or dementia

ASTHMA

General Services

Our non-chronic healthcare services include:

  • An annual flu Immunisation programme to protect the elderly and at risk

  • Regular monitoring, by blood and urine tests for patients

  • Immunisation for adults in relation to travel (not all travel immunisations are available on the NHS, please ask our practice nurse for details)

  • Routine immunisations of children, together with the Health Visiting Team.

  • Child health surveillance, alongside the Health Visiting Team where needed

  • Contraceptive services. Specialist Services such as IUCD insertion are available

  • Maternity services during ante-natal and post-natal periods, alongside the midwives from the local relevant hospitals.

  • Cervical smears

  • In-house conselling service for patients suffering from anxiety and anger management
